The Department of DME has undertaken the exercise of updating the service particulars of all Doctors working in various institutes in the State. There is an existing data base of the same with this office.
Towards achieving high degree of accuracy in compiling the data base opportunity is given to all the doctors to verify the particulars recorded in the present data base.

You are requested to type in your name only. Do not type "Dr" against the relevant text box.
Select the Institute where you are working from the list given below. Then click on submit.
There will be a display of all the data available with this office bearing the name submitted.
In the event of any errors found please click on Edit Button.
Type in the corrections to be made and click on submit.
If your name is missing please click on new form, fill the text boxes with full particulars or information and click on submit.
Note:- The additions / corrections made should be supported by documentary evidence and be forwarded to this office through proper channel for authentication.

Please fill up all the text boxes supported by documentary evidence like Service Register.
The entries should be made only by an authorized person under the supervision of the Asst. Director OR other responsible Officer of the Institute concerned.
The Head of the Institute is responsible for authenticated entries to be made regularly.
Responsibility of maintaining the password lies with the Asst. Director.
The Principals of Medical Colleges should ensure that the data of allied hospitals is also obtained and uploaded.
