1) Attested SSC Memorandum of marks to be submitted along with this application form.
2) Eligibility for appearing examination :-
For Certificate Course : The candidate must complete 12 months of training
including Practical and Theory.
For Diploma Course : The candidate must complete 24 months of training
including Practical and Theory.
3) Fee particulars: Rs.750/- for Certificate courses and 1500/- for Diploma courses.
4) For Supplementary candidates:
Certificate Courses : Rs.400/-
Diploma Courses : Rs.400/- for one subject
and Rs.750/- for 2 or more subjects
D.D to be taken in favour of Director of Medical Education, Koti, Hyderabad.A.P
5) The candidates will have to appear for the common examination from the nearest Govt. Medical College. All the candidates of one Institution will be allotted to the examination to one Govt. Medical College only.
6) Last date for Submission of application is before 5 P.M
1) Without the Hall Ticket No candidate is allowed to the Examination Hall.
2) Candidates should write their Hall Ticket Number only and not their names on the answer scripts.
3) Candidates are prohibited from writing anything either on their Hall Tickets or on question papers strict silence should be maintained in the Examination Hall.
4) Candidates should report in the examination hall 15 Minutes before commencing the examination. Those candidates who come after half �an hour of the commencement of examination shall not be allowed into the Examination Hall.
5) No candidate should be allowed to leave the examination Hall till the Expiry of Half an Hour of commencement of Examination. Once the candidate leaves the examination hall he shall not be allowed again to the examination Hall under any circumstances.
6) Candidate may bring with him pen, pencil, eraser, color pencils/sketch pens and scales.
7) The candidates should produce the practical chart books duly signed by the authorities at the time of Practical Examination.
8) Candidates should write details of examination in the first page of the Answer Script properly & completely. If they write their hall ticket number wrongly, their paper will be devalued.
9) Candidates who wrote illegibly unknown language, requests for pass marks in the answer script will be considered disqualified.
Note: The Hall Ticket is issued provisionally. If any information furnished by the candidate is found to be correct at a later date, the registration automatically stands cancelled. If impersonation or fraud is detected during and subsequent to the examination by the administration, his /her candidature is liable to be cancelled.
Action will be initiated against the Institute�s Management for forwarding his/her application illegally.
